Every safety report tells us something—not just about what happened in the operation, but about how well people were prepared for it.  This session explores the connection between operational safety reporting and training through a simple cycle:  OPERATION → REPORT → LEARN → TRAIN → OPERATE. The focus is on how lessons from the operation can inform training and how training can, in turn, better prepare crewmembers for real-world situations.

This session introduces Root Cause Analysis and how it can be applied to everyday problem-solving and operational improvement. Participants will work through a negative outcome, identify the underlying root cause, propose corrective actions, and consider how to evaluate whether those actions were effective.

The session will also introduce practical Root Cause Analysis tools, including the 5 Whys and Fishbone diagrams, that can help teams move beyond symptoms and get closer to the real problem.

What if ChatGPT could help us not only work more efficiently, but also reflect on how we think, communicate, and lead?  

This session explores how custom GPTs can turn established professional-development frameworks into personalized, repeatable self-assessment tools. Using a custom Question Mix Assessment GPT, the presenter will demonstrate how AI can identify patterns in the questions we ask, surface potential biases, and help us become more intentional in meetings, emails, and facilitated conversations.  

Participants will also see how a custom GPT can be created by translating an idea or framework into instructions, configuring the tool, and refining it over time.  The session offers a broader look at generative AI: not just as a tool for producing work, but as a tool for improving how we work — and for helping us ask better questions.
